Community Service in PS 107 Afterschool Program
The PS 107 Parents’ Advisory Committee announces Community Service in the After‐school Program. Middle school students who are PS 107 graduates may apply to volunteer for Community Service to help the After‐school teachers and staff.
Middle school students participating in the Community Service Program will learn through experience about the responsibilities and benefits of public service in a safe and fun atmosphere. Community Service activities may include reading to younger students, playing board games or sports, and assisting teaching artists with their classes.
Qualifications
Community Service students serve as excellent role models in the PS 107 school community. Candidates for the Community Service Program should be good at helping younger children and must be willing to take direction from After‐school Program staff.
Registration
There is no cost to participate in the After‐school Community Service Program. All Community Service student volunteers must apply by completing a registration form (see page 2). Registration is required to ensure that volunteers have parental permission to be at PS 107 and to ensure that the After‐school Program staff has current contact information in case of an emergency. Only registered middle school students are permitted to be at the PS 107 After-school Program.
Schedule
Community Service volunteers will be asked to commit to a set schedule of days and times of service. Attendance will be recorded. Volunteer students will sign in and sign out of the Community Service log book each day. The log book will be located in the school office.
Behavior
The PS 107 Community Standards of behavior will apply to the Community Service volunteers. Failure to uphold the standards will result in dismissal from the program and notification of the parent/guardian.

Parent/Guardian Agreement
By signing, I certify that I am a custodial parent or legal guardian of the child I am enrolling. I grant permission for my child to leave PS 107 unescorted (i.e., without an adult) at the end of his/her Community Service for the day.
Medical. I represent that the child I am enrolling in Community Service is healthy and fully able to participate in After-school Program activities. I understand that no medications will be administered by the After-school Program unless specifically authorized and in compliance with school requirements. In the case of a medical emergency, I hereby give permission to the physician selected by the After-school Program to hospitalize, secure proper treatment for, including ordering injections, anesthesia or surgery, for the child named above. The After-school Program will contact the parent/guardian as soon as possible in the event of an emergency.
Behavior. The After-school Program seeks to uphold the Community Standards established as the code of conduct at PS 107. if my child cannot live within the PS 107 Community Standards, or if my child’s behavior adversely affects the experiences of other children, I understand that my child may be dismissed from the After-school’s Community Service Program. I have discussed the policies of the After-school Program with my child, and my child understands that a consequence of his or her unacceptable behavior could be dismissal from the After-school’s Community Service Program. I have discussed in detail with my child my expectations and have stated that we, as parent/guardian and After-school Program staff, are in agreement with respect to the consequences for failure to uphold the PS 107 Community Standards.
